TBE Annual Meeting on May 26

Please complete this registration form to attend the Annual Meeting in person at Temple Beth El (TBE) at 7:30 p.m. on May 26, 2021.
A separate form MUST be submitted for each and every congregant age 12 or older planning to attend the meeting. Children under the age of 12 who are unaccompanied by an adult are also required to submit a form.
We strongly recommend that you consider not attending in person if you have any chronic medical conditions, including diabetes, hypertension, heart, lung, kidney or liver disease, neurologic disorders, immuno-deficiencies, or severe obesity. Please see the CDC website for more information about those who are most at-risk.
By attending the meeting at TBE, you do so at your own risk and agree to follow all rules and instructions.
Protocols and Requirements
- All attendees age 5 or older must have been either fully vaccinated against COVID-19 (i.e., have taken the last required dose of the vaccine) or test negative for COVID-19 within 72 hours of the meeting.
- As per CDC guidelines, everyone above the age of two must wear a face covering that covers both mouth and nose and must continue to wear it while on TBE premises, including during the meeting. Coverings must be provided by the individual and will not be made available by the synagogue.
- All attendees must maintain 6 feet of distance from all others, except for immediate members of a family.
- There should be no physical contact between attendees, except among immediate family members. This includes hand shaking (even with gloves), fist bumps, hugs, and kisses.
- The main-floor bathrooms will be available for use, with entry permitted for two occupants at a time. The rest of the premises at TBE is off limits.
- Attendees are requested to use the hand sanitizer upon entry to the building.
- The use of cell phones and other electronic devices is strictly prohibited.
- Attendees must remain at their seats for the duration of the meeting except if they are a participant (e.g., speaker, presenter, award winner), need to use the lavatory or wish to leave early.
- Attendees must adhere to all directives given at the meeting and follow directions for exiting once the meeting concludes.
- Attendees from out of state must adhere to the quarantining guidelines as per the Connecticut travel advisory, found here
